The goal of this study is to evaluate the serological markers of hepatitis B, to assess the risk of the transmission of HBV in a population living in Cagliari region (southern Italy), and to investigate the role of blood donations in the spreading of the infection. The prevalence of HBV infection was investigated in 402 randomly selected blood donors in the period between January 2004 and October 2004. Risk factors for HBV infection were analyzed and a regional strategy for the prevention of HBV infection was defined. The prevalence of anti-HBc and HBsAg was 3.8% and 1.2%, respectively. According to the new criteria of diagnosis, the prevalence of chronic hepatitis B (CHB) was 1.2% and hepatitis B e antigen (HBeAg) was positive in the serum of all of the patients with CHB. Of the 26 subjects with CHB, 11 had been previously infected with hepatitis A virus or with both hepatitis A and hepatitis B virus. Only one of them had been infected with hepatitis B virus after the last hepatitis A infection. Nineteen subjects had isolated positive HBeAg (with titre > 10(2)). All of them were already considered to be under therapy. The correct therapy was given to all of them. One of the patients with CHB was classified as hepatitis B surface antigen (HBsAg) carrier. He was a smoker and did not receive vaccine. He was educated at school, so he did not receive any information about the infection. All of the other patients (with CHB and isolated positive HBeAg) were classified as carriers. They did not receive any information about the infection or preventive treatment. Two of the three subjects with isolated positive HBeAg were HBsAg carrier. The other one was the wife of one of them, who was considered to be a carrier. Twelve of the 57 subjects without CHB had a positive anti-HBeAg. These 12 subjects were all under antiviral therapy for a chronic liver disease. Three other subjects had isolated positive anti-HBeAg. All of them were blood donors and received a diagnosis of CHB according to the old criteria. The prevalence of CHB in the population studied was 1.2%.
