The proinflammatory cytokines interleukin-1beta (IL-1beta) and tumor necrosis factor-alpha (TNF-alpha) are potent modulators of the function of ECs and promote inflammatory responses to vascular injury. The release of IL-8 by ECs has been implicated in the generation of the chemotactic gradient and subsequent migration of polymorphonuclear leukocytes (PMN) into inflamed tissues. We have investigated whether IL-8, a potent chemoattractant for PMN, is also secreted by ECs. ECs were incubated with IL-1beta and TNF-alpha. The release of IL-8 was detected by enzyme-linked immunosorbent assay. TNF-alpha caused a dose- and time-dependent release of IL-8 from the EC. The TNF-alpha-induced IL-8 release was completely inhibited by anti-TNF-alpha monoclonal antibody. Recombinant IL-1beta also stimulated the release of IL-8 from the EC. The IL-1-induced IL-8 release was inhibited by anti-IL-1beta antibody and IL-1beta receptor antagonist but not by anti-TNF-alpha antibody. These results indicate that IL-1beta and TNF-alpha can stimulate the release of IL-8 by ECs. The TNF-induced IL-8 release was mediated by both TNF receptors. These data support the possible physiological relevance of IL-8 and suggest that the TNF-alpha and IL-1beta may contribute to PMN migration into the vascular wall during inflammatory states.

PROM was only associated with maternal and intrapartum risk factors in only 20.2% of cases. The risk factors that increased the incidence of PROM were maternal age > 40 (RR 6.3) and > or = 7 days interval between the last prenatal visit and PROM (RR 4.7). The incidence of PROM is increasing. Our findings show that a significant number of cases of PROM are due to primary PROM of greater than 30 weeks of gestation. The implications of these findings are discussed.. The instant case is an appeal from an order denying a motion for sanctions in a medical malpractice suit brought under Chapter 10 of the Texas Civil Practices and Remedies Code. We are confronted only with issues arising in the course of the hearing on the motion for sanctions.
